In addition to his parents, he is survived by his two sisters: Ruby and Scarlet Chapman of Madisonville, KY; his brother, Phoenix Chapman of Madisonville, KY; his maternal grandparents, Edith Utterback of Dixon, KY and Mike and Mary Utterback of Hopkinsville, KY; his paternal grandparents: Thomas and Mary Chapman of Fort Myers, FL and Susan Gilbert of North Glenn, Co.

Funeral services will be private.
